About the Medicaid Primary Care Program

The Medicaid Advanced Primary Care Program is part of Maryland's broader AHEAD (Achieving Healthcare Efficiency through Accountable Design) Model.

The program focuses on strengthening primary care through:

  • Improved access to comprehensive primary care services
  • Proactive identification and management of high-risk patients
  • Better care coordination and transitions of care
  • Addressing social determinants of health (SDOH)
  • Improved preventive care and patient engagement
  • Enhanced quality reporting and performance improvement
  • Greater alignment between Medicare and Medicaid care transformation

Participating practices must navigate clinical, administrative, data exchange, and care transformation requirements.

Netrin helps translate these requirements into practical, sustainable workflows.

Who Is Eligible?

  • Maryland-based primary care practices or a Federally Qualified Health Center (FQHC).
  • Practices that are using a certified EHR
  • Practices that have a panel size of at least 250 HealthChoice (Maryland Medicaid) members
  • Must plan to co-participate in both Medicare and Medicaid AHEAD programs by 2027.

How it Works

To support care transformation efforts, Maryland’s Medicaid Advanced Primary Care Program (MAPCP / Medicaid Path) offers participating primary care practices transformative new revenue streams

  • Care management fees every quarter
  • Performance-based incentive payments every year
  • Increase of Medicare PFS on every eligible office visit claim

Testimonials

"The support we receive from Netrin is invaluable. The Medicaid AHEAD reporting requirements are complex and very labor intensive. We simply don't have the internal resources to manage all the reporting, regulatory compliance, and care"

“Netrin has really helped us to modernize our practice by adding new modalities that enhance our day-to-day patient care. They used updated technology with remote patient monitoring devices which allow us to keep a handle on patients with chronic health issues. The process of getting that data connected well and integrated saves us a lot of time while avoiding a lot of complications.”

"Netrin helps primary care providers stay independent by letting them come together and share resources, providing the support that they do not have internally. Having an organization like Netrin helps us take advantage of additional services and support mechanisms that enhance our patient care, which has been really the key to our success in value-based care."
