Netrin Health ranks #1 in the key program metrics including ‘Emergency Department Utilization’ and ‘PQI-Like Events among 21 other Care Transformation Organizations.

As an approved Care Transformation Organization (CTO) within the Maryland Primary Care Program (MDPCP), Netrin Health continues to support primary care providers make transformative changes to the way they deliver care.

The Maryland Primary Care Program (MDPCP) is a voluntary, advanced primary care program that is jointly operated by the Maryland Department of Health and the Center for Medicare and Medicaid Innovation (CMMI). The primary goal of the MDPCP Program is to strengthen primary care for promoting health and reducing overall health care costs across the state of Maryland.

Since the program launched in 2019, Netrin Health has been providing primary care providers with the tools, technology, and resources that they need to achieve a successful health care transformation process, by providers them play an increased role in prevention, management of chronic disease, and preventing unnecessary hospital utilization. 

Netrin Health ranks #1 in ‘Emergency Department Utilization’ and ‘PQI-Like Events’, and rank #5 in ‘Inpatient Utilization’ among 21 other CTOs.

The key metrics results announced for 2021 are proof of Netrin Health’s outstanding performance and progress as a CTO to help providers achieve the strategic goals of the MDPCP. The following graphs contain 2019 vs. 2021 performance data for organizations participating in the Maryland Primary Care Program as CTOs. The data is based on performance of each CTO’s partner practices on key program metrics.

Netrin Health as an MDPCP CTO

Netrin Accountable Care has been an approved MDPCP CTO since the program launched in 2019 and serves all Maryland counties. As of 2022, Netrin CTO has successfully transformed 100% of its partner practices as an MDPCP Track 2 participant.
